Contemporary institutions distribute action across specialised roles, procedural stages, and unequal levels of authority. This division enables complexity but can also separate individual agency from collective consequence. The resulting “accountability gap” is often treated as a problem of insufficient personal virtue. This essay argues instead that institutional answerability requires a moral architecture: explicit distinctions among causal, role-based, moral, and remedial responsibility; protected routes for disclosure; independent fact-finding; proportionate judgment; and organisational memory capable of converting failure into repair. The central question is not whether responsibility is individual or collective, but how institutions can preserve answerability when agency is distributed.
